We are currently in-network with the following commercial insurances BCBS, Aetna, Aetna State Health Plan, United Healthcare, Optum, and UMR.

Please note: We do not accept any Medicare or Medicaid insurance, even if affiliated with BCS, Aetna, or UHC. Please speak with our billing staff if you have one of these policies, before your appointment, to understand payment arrangements.





Please note: Carl Credle has applied for credentialing with insurance providers and is awaiting approval. Until credentialing is complete, Carl will be accepting patients on a self-pay basis.

Please note that self-pay rates differ by provider; Dr. Brown and Mr. Credle each have their own pricing based on their clinical roles. Payment is due at the time of service.
We understand that you have a life with its own commitments and that unexpected changes can arise. With more than 24 hours notice, we are happy to try to accommodate your schedule to the best of our ability.
If you need to cancel or reschedule a session, please provide at least one business day (24 hours) notice. For example, appointments scheduled for a Monday must be cancelled on or before the preceding Friday.
Effective November 1, 2025, if you miss a session without canceling or cancel with less than 24 business hours’ notice, you will be charged the $200 for missed new patient appointments and $100 for missed follow-ups. This charge must be paid before a new appointment can be scheduled.
Please note: insurance companies do not provide reimbursement for late cancellations or no-shows.
If you are uninsured or choose not to use insurance, you are entitled to a Good Faith Estimate explaining costs. You may request a GFE at any time.
We can file out-of-network claims on your behalf; however:
Patients may also choose not to file insurance and pay fully out of pocket, understanding this will not count toward their deductible.
